As Matt said pay the price on ebay it is a big hassel without the stock plate to tread-in. Thank you, FAA Univair  will not just send you one. And how can anyone prove the airframe is truly what you say it is                   The Date of Manufacture is the middle field...

The Univair folks have a blank plate, as do several Ebayers.
There are three fields for information- 1. serial number, 2. ?  and 3. Horsepower.
 

Does anyone know what information goes in the middle field, if any?

that is not the data plate from Erco, that might be put under the horizontal, but even there it does not meet spec, which is made of stainless steel. There is a FAA Advisory circular that tells how to go about getting a new data plate. There are blanks on Ebay all the time as well

Hello all,
Is this a proper data plate? It's what came with the papers, and it matches the AWC but it was adhered somewhere (not riveted). There is dry adhesive on the backside. It was presumably removed when it was stripped and polished. I don't have any photo of the pilot side aft to confirm it was ever attached under the horizontal stabilizer, which is where I assume it should be?
